Jun. 1--QUESTION: A doctor performed a cardiac catheterization on my mother. I have written several letters, sent by certified mail, to the doctor and others requesting a copy of the cardiac catheterization report.
The only response I've received was a handwritten note from the doctor stating, "Report is dictated."
Why have I not received a copy of the report he says he dictated?
